Does anyone have an issue with rear light condensation like this?

Is the lens completely blown, or is there a fix that can be done?

Yes. Most do. It's due to the design. The back is open and inside the car, where the warmer air enters the light. As the clear part is exposed to the cooler outside air, you get condensation.
